• Federal Student Aid ID (FSA ID) — get one at StudentAid.gov
• Your demographics — gender, race, age, citizenship status, parents’ marital status
• Parent / Legal guardian’s email
• Names of schools to receive report

• Your most recent tax return (if applicable)
• Unofficial transcript
• Personal statement — often required for specific scholarships
• Letters of recommendation
• Creative portfolio / Writing sample — required for some awards

College Applications
(including the Common Application)

• Parent / Legal guardian’s personal info — employment, education (include degrees earned), contact details
• Household income — if requesting application fee waivers
• Your current class schedule
• Personal email address — do not use your high school email

• List of academic honors and awards — may list up to five
• SAT / ACT / AP / IB test scores — if you didn’t send reports directly to the Common App or this school
• List of high school activities — include jobs, volunteering and co-curriculars
• Unofficial transcript
• Personal statement and other essays
• Creative portfolio / Writing sample — required for some programs
